Cardiac Care
Mercy Home Care's intensive management of heart patient
cases has resulted in exceptional clinical success. We've
reduced our patients' post-hospital readmittance rate
for congestive heart failure from the national average
of 20 percent to eight percent.
Orthopedic Care
In this unique program, our orthopedic case manager coordinates
all patient needs, from home assessment and education
to specialized cross-training of staff. For example,
we've trained orthopedic nurses and therapists in a
variety of post-operative procedures to enable patients
to leave the hospital sooner.
Infusion therapy
Most infusion therapy needs can be met at home. Mercy Home
Care is a single-source for pharmaceutical, equipment
and RN care. We coordinate all elements of infusion therapy
with key hospital personnel, providing home assessment,
patient care assessment and initial patient and caregiver
education.
Our multidisciplinary team of specially trained and certified
pharmacists, infusion RNs, dietitians and reimbursement
specialists work with the patient's physician and other
needed services to provide comprehensive care. We offer
a variety of treatments including:
- Pain control
- Hydration
- Central line care
- Chemotherapy
- Midline line insertion
- Antibiotics
- Total parenteral nutrition
- IV push medications
- Steroids
- Anticoagulant therapies
